You don't have any other mods enabled which assign land to Japan/China/Germany/etc.?

As the Country Overhaul option should remove all claims, cores, and controls from states; though if another mod adds those back (or they're added back by an event, for example) then they would reappear.

No i did not have any other mods active except for mine but i did check the states in the mod folder and it does say 

Owner = ITA

On albania for example
